Do you take insurance?

How much is it?
The low monthly fee depends on your age (see pricing page)
Are there Co-pays?
No. We do not bill insurance companies. There is only a monthly fee from our patients for unlimited office visits and other services provided by our office.
Is there a sign-up fee?
Yes. It is a one-time fee of 75 dollars.
Do you draw blood in-house?
Yes. We draw the blood at the office and it is picked up by courier and taking to the laboratory for processing. These test costs are reduced for our members.
What about CT and MRI referrals for cash-pay patients?
There are reduced cash prices available for CT, MRI, ultrasound, and x-ray studies at several locations in Lake City.
Can you do referrals?
Yes. We can refer to specialist. If you have insurance, we may be able to refer you to an in-network provider (depending on your insurance policy)
What about medications?
We have an in-house pharmacy to provide low cost medications to our clients. We can also send your prescriptions to the pharmacy of your choice.
